AWARDS DAY REPORT

Seniors or their parents are asked to complete this report so that we will have accurate information for our May awards day, which will take place on Tuesday, May 14. We rely on information from students and parents since many colleges do not share this information.

Note that we will not share need-based awards such as PELL grants. We are looking for merit scholarships & awards based on your scholastic record, tests, volunteer involvement, athletic or fine arts involvement, and so forth.

There are three sections to the report. You might use only one section, but please consider all three as you complete it. Be as specific as possible (include name of scholarship, amount, renewable?, etc.)

1. School Scholarships & Awards - Share everything offered—even from schools not attending
2. Scholarships/Awards from other entities - private organizations, parents' or student's work, your church, etc.
3. Special awards or recognition - non-monetary awards like acceptance to a special program, honors college, apprenticeship, internship, etc.
